OFMS 7th & 8th Grade Cross Country Information

Cross country begins on Monday, August 1. 

Athletes must have physical forms and concussion forms completed or already be current and on file for athletes to be eligible to practice on August 1. Physicals are considered current for 365 days. Further, the 365 days must encompass the full cross country season, which ends in mid October. While a physical is considered current for one calendar year, a concussion form must be completed for each athletic season. 

Again, athletes are not able to begin practice on August 1, unless we have a current physical form and a concussion form on file. 

Please make sure your child’s doctor completes all appropriate parts of the physical form and all signature sections are signed. Incomplete physical forms and missing signatures will result in your child not being able to begin practice until all sections are completed. It is recommended that you check over the physical form before you leave the doctor’s office. Health screenings at med centers constitute a physical as long as the form is completed thoroughly.
