Attention all Olmsted Falls Student – Athletes in current Grades 6th–11th:
YOU NEED AN OHSAA PHYSICAL EVALUATION FOR THE 2016-2017 SCHOOL YEAR!
Avoid making a separate appointment with your doctor, or waiting in line at the local clinic. We will have more doctors this year to make the process run faster.
When: May 25th, 2016 from 4:00PM – 7:00 PM
Where: Olmsted Falls High School – Main Gym & Locker Room area
NO APPOINTMENT necessary
Cost: $20.00. Cash or checks accepted.
Checks made payable to our “Olmsted Falls Athletic Boosters”
Sign-ups: We will use an online sign up service to select a time slot.
Please bring the following;
1. Money (Check made payable to the OFAB or Cash)
2. An OHSAA Physical Form with ALL of your personal information completed, to save time! Physicals can be found in the OFHS or OFMS office area, or on the district website.
Parents MUST sign the form before your physical can be given.
ALL PROCEEDS FROM THIS EVENT WILL BENEFIT OUR ATHLETIC BOOSTER ORGANIZATION!
