Washington DC Information – 8th Grade Parents

The following represents information about upcoming events as well as details and forms about our upcoming trip to Washington DC.

  1. All 8th graders will make a visit to Polaris Career Center which will serve as a general overview of their program and services. Both 8th grade teams will be traveling by bus to Polaris on Bagley Road in Middleburgh Heights on Wednesday, February 3. The students will eat lunch at school at the regular time on this day, which is from 11:00-11:30 AM. The Gold team will travel to Polaris from 8:40-11:00 AM, and the Blue team will travel to Polaris from 11:30-2:00 PM.

  2. Parent chaperones who have been selected to attend the trip with us in April will be notified by the end of the school day on Monday, February 1, 2016. We had over 100 parents express and interest and are only able to take 18-20 parent chaperones. If you signed up and do not get notified, please understand that you were not selected. I apologize for this up front as I always feel bad when we are not able to select all of our volunteers. It is amazing that we have so many parents who are able and interested in attending this trip, which is a credit to our school and to our community.

  3. All of our Washington DC forms are electronic this year. Please click on the link below to complete all required forms. Forms must be completed online by Friday, February 5, 2016.

  4. The medication forms are still in paper form and are attached. If you have any questions about medication please contact Mrs. Koenig, our Clinic Health Aide, at 440-427-6212, or by email at jkoenig@ofcs.net. Mrs. Koenig will contact our district registered nurse for consultation if additional information is needed. Medication and medication forms are due to the office by Friday, March 11, 2016.

  1. Our Parent Luggage Search Form is in paper form and is due the morning of the trip after you have checked your child’s luggage before you depart your home. The Luggage Search Form is attached.

  1. Our final parent and student DC meeting will be held from 7:00-7:45 PM on Wednesday, February 3, in the OFMS Cafetorium. We need at least one parent to attend with their child.

  2. Final payment is due by Friday February 5. Payments will be collected during 8th grade lunch during the week of February 1-5. Payments may be made at the parent meeting, as well. Payments may be made in cash, checks (payable to OFMS), and online through EZPay @ www.spsezpayolmstedfalls.com

  3. Final balance sheets will be sent home with all students on Friday, January 29, and will include updated information from our most recent fundraiser.
