Avoid making a separate appointment with your doctor, or waiting in line at the local clinic.
GET YOUR PHYSICAL RIGHT AT OLMSTED FALLS HIGH SCHOOL!
When : May 21, 2014 from 2:30PM-5:30 PM
How Much : The cost is $20. Cash or checks made out to “OFAB” or “Olmsted Falls Athletic Boosters” will be the only forms of payment.
Bring:
1. Money (Check or Cash)
2. An OHSAA Physical Form with ALL of your information filled out to save time!
**If you are under 18, parents MUST sign the form before your physical.
ALL MONEY WILL GO TO OUR ATHLETIC BOOSTERS!
